Home and Community-Based Services (HCBS) were first introduced in 1983 when Congress added Section 1915(c) to the Social Security Act, allowing states to apply for waivers from Medicaid rules that prioritize institutional care.
Who We Are
HomePlus Care is a 501 (c) 3 tax-exempt nonprofit organization and Home and Community Based Services provider. Validated by the Office of Long Term Living of PA Department of Human Services, we serve individuals living in Philadelphia, Bucks, Delaware, Montgomery, and Chester counties.
We provide Personal Assistance Services as coordinated by Managed Care Organizations (MCOs), who operate health care services under contracts with the PA Department of Health and the PA Department of Human Services via Community HealthChoices Long Term Services and Supports of Medicaid Waivers (Managed Care Waivers and Home and Community Based Services Waivers).
For eligible individuals, we assist with Activities of Daily Living (ADLs) and Instrumental Activities of Daily Living (IADLs) as part of our Personal Assistance Services. These include bathing, dressing, eating, transferring, toileting and continence & incontinence (ADLs) and preparing meals, housekeeping, shopping, handling transportation, managing medications, using communication devices and managing finances (IADLs).

Our Direct Care Workers are here to provide caring, personalized support to older adults and individuals with disabilities, helping them stay active and independent in their own homes and
communities. Through Pennsylvania's Community HealthChoices (CHC) Long-Term Services and Supports (LTSS) Home and Community-Based Services (HCBS) program, these dedicated team members assist with
activities of daily living and instrumental activities of daily living (personal care, daily routines, mobility, etc.) and connecting to local activities and resources. With a focus on safety,
dignity, and respect, our Direct Care Workers are committed to enhancing quality of life and ensuring each person feels valued and supported within their community.
These services aim to support individuals in actively participating in their communities, enhancing their independence, and fostering social connections. Through tailored assistance, individuals receive support in accessing community resources, engaging in meaningful activities, and building relationships. The program is designed to empower participants to live more inclusively, supporting a high quality of life within community settings rather than institutional environments.
