TB screening is a critical component of the health and safety protocols for Direct Care Workers under PA Community HealthChoices Long-Term Services and Supports. Ensuring compliance with these requirements helps protect both care workers and the individuals they serve from the risks associated with TB infection.

What is Tuberculosis?

What is Tuberculosis (TB)?

Tuberculosis (TB) is a contagious bacterial infection caused by Mycobacterium tuberculosis. It primarily affects the lungs but can also impact other parts of the body. TB is transmitted through airborne droplets when an infected person coughs, sneezes, or speaks.

Why is TB Screening Important for DCWs?

Direct Care Workers (DCWs) providing services under Pennsylvania's Community HealthChoices (CHC) Long-Term Services and Supports (LTSS) program work closely with vulnerable populations, including seniors and individuals with disabilities. TB screening is crucial to prevent transmission and ensure a safe care environment.

Mandatory TB Testing Requirement for DCWs

Under PA CHC LTSS guidelines, DCWs must undergo TB screening as part of their pre-employment health assessment. The specific requirements include:

1. Initial TB Test (Pre-Employment Screening):

  • A Mantoux tuberculin skin test (TST) or an Interferon-Gamma Release Assay (IGRA) blood test must be performed before employment begins.

  • If the test is positive, the worker must undergo further evaluation, including a chest X-ray and medical review, to rule out active TB.

2. Annual or Periodic Screening:

  • Additional TB screening may be required depending on employer policies or exposure risk.

  • Workers with known positive TB tests must provide documentation of past evaluations and undergo routine symptom screening.

3. Documentation and Compliance:

  • Employers must maintain records of TB test results and ensure compliance with state health regulations.

  • Non-compliance may result in employment restrictions or required medical clearance before returning to work.

Recommendations for Direct Care Workers:

  • Report any TB symptoms (persistent cough, fever, night sweats, weight loss) immediately to a healthcare provider.
  • Follow all infection control measures, including the use of personal protective equipment (PPE) when necessary.
  • Stay up-to-date with employer-mandated health screenings.
